The goal of It's Your Game: Keep It Real is to reduce teen pregnancy, prevent STI transmission, and delay teen sexual activity in middle school students.
Participants in the It’s Your Game: Keep It Real intervention program were less likely to initiate sex by the ninth grade when compared to the control group.

The John Hopkins Community Health Partnership's (J-CHiP) goal is to improve care coordination with Medicare and Medicaid beneficiaries.
The John Hopkins Community Health Partnership participants saw lower spending and improved health outcomes in regards to hospital admissions, re-admissions, and emergency department visits.

The goal of Kinship Connections is to support kin families' social, emotional, and economic needs to increase placement stability within the child’s community. The mission of this program is to improve the lives of individuals and families affected by alcohol and other drug use through treatment, education, and research.
Matrix participants were 38% more likely to stay in treatment and 27% more likely to complete treatment compared to nonparticipants. Stimulant drug-use indicators were significantly reduced during treatment for Matrix participants. They also produced more drug-free urine samples compared to nonparticipants.

The Minority AIDS Project's mission is to reduce suffering and deaths due to HIV infection in the African-American and Latino communities by making HIV/AIDS education and related health services available and accessible.
The Minority AIDS Project provides services and outreach efforts to predominantly African-American and Latino individuals living with HIV/AIDS in the underserved communities of Central and South Central Los Angeles.

The goal of Parent Connext is to support parents in reducing and/or preventing toxic stress in the family and help children develop critical life skills and coping skills. Recent studies have found that up to 50% of health outcomes are attributable to social and economic factors and that lifetime costs associated with child maltreatment are comparable to other costly healthcare conditions such as stroke or type 2 diabetes. Moreover, 4 in 5 physicians report lacking confidence in their ability to meet patients’ social needs, which can impede their ability to provide high quality medical care. As a result, interventions that target parents’ social needs may have important implications for reducing healthcare costs and have the added benefit of enabling physicians to provide high-quality care to their patients.

The goal of Pathways to Housing, Inc. is to provide independent housing, treatment services for physical and mental health, and employment to mentally ill individuals who are chronically homeless.
Pathways to Housing has received numerous awards for its projects in New York City, Philadelphia, and D.C. In addition to achieving large effects in terms of housing outcomes, compared with standard care in the community, Housing First also improves community functioning and quality of life.
